Carl has been serving seniors for more than 30 years. He has a diverse background in hospital administration, long term care, assisted living, independent senior living, community-based support services, adult foster care, facility management, hospice/palliative care and home care.
Carl is well-versed in all senior support services in Michigan. He holds a Bachelor of Sciences in Health Care Administration from Eastern Michigan University.
Melissa received her Bachelor’s in Social Work from Saginaw Valley State University. She has worked at the Saginaw County Commission on Aging and various Rehabilitation and Nursing Homes, giving her extensive experience with care plans, discharge planning, and one-on-one interaction with the geriatric population. Everyone who knows Melissa says she’s the world’s best listener and has very realistic advice.
Sarah received her Bachelor’s degree from Valparaiso University and her Master of Science in Social Work from the University of Texas. She also holds certification in Advance Care Planning. She has worked as a case manager and inpatient social worker in the Henry Ford Health System and has been instrumental in initiating, facilitating and managing support groups, as well as connecting patients tor various resources. She also has extensive experience working with insurance services.
Matt is in court every week representing our guardianship and conservatorship clients. So, he has great relationships with judges and attorneys in Oakland, Macomb, Wayne, Livingston and Washtenaw counties. His strong background as a nursing home social worker makes him an expert in Medicare and Medicaid reimbursement, resident’s rights and discharge planning and placement.
Matt earned his Bachelor of Science in Health Science and Sociology. He is also holds a State of Michigan Nursing Home Administrator License.
Josie received her undergraduate degree at Siena Heights University, and her Masters in Social Work at Wayne State University. She has worked as a discharge planner, case manager, counselor and Director of Social Work in nursing homes, assisted living facilities and health centers. This experience gives her the knowledge to answer any questions regarding discharge planning, skilled nursing services, private duty services, counseling services, care management and power of attorney / guardianship / legal / insurance questions or concerns.
Josie has more than 20 years experience working in diverse settings with 11 years of long-term care experience within a medical setting.
Traci has been a nurse for more than 24 years, specializing in clinical care management with significant participation in care plan development for those with Gero-psychiatry needs. She also has experience as a Medicare Manager in a senior facility and has managed a 50-bed rehab unit. Traci received her RN from Oakland Community College, and holds infusion therapy certification. Her wealth of experience in homecare and rehab includes clinical care and ventilator care. For the past decade, she’s shared her expertise with Senior Care Network.
Traci has successfully partnered with hospice, local hospitals and discharge planners to bring her patients the very best outcome. Her dedication to seniors led her to co-found a non-profit which celebrates life for the elderly.
